The patient needs immediate coverage with insulin for hyperglycemia. Choice a  provides a very long acting insulin. Choice c can NEVER be given intravenously. Choice d is the distracter, it could be used but it will not be effective in controlling a blood glucose of 304 mg/dl immediately.
