DISCUSSION:

The clinical presentation is consistent with an acute inflammatory process in the topography of the right lower quadrant. The most likely diagnosis is acute appendicitis. A STAT CT of the abdomen and pelvis with oral contrast only is indicated to evaluate this patient.
